The cardholder form for employees (to be completed and signed by both the employee and employer) can be forwarded either by mail or facsimile to PBI on 1300 400 514 or Westpac on (02) 9374 7916 – attention Team Leader, Commercial Cards New Business Unit: (retain the fax confirmation from Westpac for your records)
