How do I apply?

It's easy. Simply:

 

  1. Complete the Salary Sacrifice Agreement, available here.
  2. Complete the Everyday Purchase Card Application Form (including the Direct Debit Request Form), available here.
  3. Return the completed forms to your employer, who will then liaise with PBI Solutions to complete your card application.
